Private Pay Transportation Services
Reliant Medical Transport currently provides Non-Emergency Medical Transportation (NEMT) services on a private-pay basis. Payment is required for all transportation services and may be made using approved payment methods at the time of scheduling.
Future Medicare and Medicaid Participation
Reliant Medical Transport is actively working through the credentialing and enrollment process to become an approved provider for Medicare and Medicaid transportation services. Our goal is to expand access to reliable transportation for individuals who rely on these programs for their healthcare needs.
While we are not currently able to bill Medicare or Medicaid, we look forward to offering these options in the future. Updates regarding our participation status will be posted on our website as they become available.

This notice explains how your medical information may be used and disclosed, as well as your rights regarding that information and how you can obtain access to it. Please read it carefully.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Reliant Medical Transport ("Reliant") is required by law to maintain the privacy and security of your Protected Health Information ("PHI"), provide you with this Notice of Privacy Practices, and comply with the terms of this Notice currently in effect.
Reliant Medical Transport will notify affected individuals following a breach of unsecured Protected Health Information when required by applicable federal and state law.
USES AND DISCLOSURES OF PHI
Reliant may use and disclose your PHI for treatment, payment, and healthcare operations without your written authorization.
Treatment
We may obtain information about your medical condition and transportation needs from you, your healthcare providers, caregivers, health plans, or other authorized individuals in order to coordinate and provide transportation services. We may share PHI with healthcare providers, facilities, dispatch personnel, and others involved in your care and transportation.
Payment
We may use and disclose PHI to bill and receive payment for transportation services from Medicaid, Medicare, managed care organizations, commercial insurers, transportation brokers, or other responsible parties.
Healthcare Operations
We may use PHI for quality assurance, employee training, licensing, accreditation, auditing, compliance monitoring, customer service, and other business operations necessary to support our services.
Transportation Coordination and Electronic Communications
Reliant utilizes electronic dispatch systems, scheduling software, GPS technology, mobile devices, telephone communications, secure email, and other technologies to coordinate transportation services. We may contact you regarding scheduled trips, appointment reminders, service updates, cancellations, delays, or transportation-related information.
USES AND DISCLOSURES WITHOUT AUTHORIZATION
Reliant may use or disclose PHI without your written authorization when permitted or required by law, including:
• Treatment, payment, and healthcare operations.
• Public health activities.
• Reporting abuse, neglect, or domestic violence.
• Health oversight activities, audits, investigations, and inspections.
• Judicial and administrative proceedings.
• Law enforcement purposes.
• Military and national security activities.
• Workers' compensation claims.
• Coroners, medical examiners, and funeral directors.
• Organ procurement and donation activities.
• Research approved under applicable legal requirements.
• To prevent or lessen a serious threat to health or safety.
• Other uses and disclosures required or permitted by law.
MARKETING AND SALE OF PHI
Reliant will obtain your written authorization before using or disclosing your PHI for most marketing purposes or any sale of PHI as required by law.
OTHER USES AND DISCLOSURES
Any use or disclosure not described in this Notice will be made only with your written authorization. You may revoke an authorization at any time in writing, except to the extent that action has already been taken in reliance on that authorization.
YOUR RIGHTS REGARDING PHI
Right to Access and Obtain Copies
You have the right to inspect and obtain a copy of your PHI maintained by Reliant. You may request records in paper or electronic format when available. We generally will respond within 30 days as required by law.
Right to Request Amendment
You may request that Reliant amend PHI that you believe is incorrect or incomplete. We may deny certain requests as permitted by law.
Right to Request an Accounting of Disclosures
You may request an accounting of certain disclosures of your PHI made during the six years preceding your request.
Right to Request Restrictions
You may request restrictions on certain uses and disclosures of your PHI. While Reliant is not required to agree to all requested restrictions, any restriction accepted in writing will be honored.
If you pay for a service in full out-of-pocket, you may request that Reliant not disclose information regarding that service to your health plan for payment or healthcare operations purposes, and we will comply when required by law.
Right to Request Confidential Communications
You may request that we communicate with you using alternative methods or at alternative locations.
Right to Receive Notification of a Breach
You have the right to receive notification if a breach of your unsecured PHI occurs and notification is required by law.
Right to Obtain a Paper Copy of This Notice
You have the right to receive a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
CHANGES TO THIS NOTICE
Reliant reserves the right to revise this Notice at any time. Any revised Notice will apply to all PHI maintained by Reliant. The current version will be available upon request and, if applicable, posted on our website.
COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with Reliant Medical Transport or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
